In your first two examples you indeed get a real LD, not a fake one, by falling asleep/dreaming *inside* your first dream. So in that case, the model is [waking life] -> [dream life] -> [real lu ...

I don't know if they are ripping you off - some small research on Dutch and English websites seems to confirm the prices you're given. 15 euro is pretty cheap - most treatments seem to start at 60 eur ...
